1. At the end of the semester each academic department will, during the last week of class, instruct the students to fill out the Student Services Survey.
2. At the end of the semester, each academic department will forward the completed survey forms to the department chairmen for compilation.
3. Department chairmen will forward a copy of the compiled student survey data to the Dean's office to analyze the question on the student services survey applies.
4. If any service department has 10% of the students answering strongly disagree or Disagree to any question; that service department chairman must meet with the appropriate Dean/Vice President to develop a plan of action to correct the problem.
