How to Access:

1. Login: Visit the system portal and enter your login credentials.

2. Select the Service:

  • Disability Status: Submit information related to the disability condition.
  • Social Case Review: Request social support or case assessment.
  • Status Card: Complete the form to obtain an identification card.
  • Confidential Case Study: Submit a confidential case related to student support.
  • Financial Assistance Request: Submit a request for financial aid or support.
     

3. Data Entry: Complete the forms accurately with the required information.

4. Review: Verify the accuracy of the entered information before submission..

5. Submit: Click the submit button after reviewing the data.

6. Track Status: Follow up on the request status through your personal dashboard.
